Obstacles à la préservation de la fertilité chez les adolescents et les jeunes adultes

Abstract


La population d’adolescents et de jeunes adultes atteints de cancer est confrontée à des problèmes particuliers liés au traitement, aux symptômes psychologiques et à l’accès à des soins adaptés. Notamment, de nombreux traitements sont gonadotoxiques et ont des répercussions sur la fécondité. Pourtant, dans ce groupe d’âge, nombreux sont ceux qui ne cherchent pas à préserver leur fertilité, ce qui a des effets négatifs sur leur qualité de vie à court et à long terme. Des patients âgés de 14 à 39 ans, traités dans un centre médical universitaire de banlieue, ont accepté de participer à une étude de registre plus vaste. Les participants ont été contactés par téléphone et ont assisté à un entretien semi-structuré sur leur expérience en matière de préservation de la fertilité, qui a été transcrit par la suite. Les entretiens ont été codés en fonction des thèmes relevés dans la littérature existante. Sur les 57 patients admissibles, 23 ont participé aux entretiens. Les thèmes les plus fréquemment codés étaient l’urgence du traitement (20,6 %) et la durée de préservation des gamètes (14,7 %). Parmi les autres thèmes figuraient le coût du traitement initial (8,8 %), le fait que les patients ou parents n’étaient pas mis au courant des dangers (8,8 %) et la perception erronée du risque de gonadotoxicité comme étant faible (8,8 %). Les médecins justifiaient le fait de ne pas proposer la préservation de la fertilité en invoquant différentes raisons, comme le fait que les patients ne voulaient pas d’enfants ou étaient déjà parents; on a toutefois constaté le contraire : ce n’est pas parce qu’ils n’éprouvaient pas le désir d’avoir des enfants (0 %) ou qu’ils étaient déjà parents (2,9 %) qu’ils ne participaient pas au processus. Bien sûr, le fort désir d’avoir des enfants (26,1 %) figurait parmi les facteurs en faveur de la préservation de la fécondité, mais aussi la bonne communication, au bon moment, avec le professionnel de la santé (21,7 %). Toutefois, de nombreux facteurs expliquent que la préservation de la fertilité soit une ressource sous-utilisée par les adolescents et les jeunes adultes. Malgré l’amélioration de l’éducation des patients à ce sujet dans l’ensemble du système de santé et l’élargissement des couvertures d’assurance obligatoires, les adolescents et les jeunes adultes demeurent réticents à suivre un traitement de préservation de la fécondité. Les résultats de recherche montrent qu’il faut continuer d’encourager la tenue de discussions conséquentes au bon moment pour promouvoir la prise de décision partagée et éliminer les facteurs qui bloquent le recours au traitement.
