Le rôle infirmier en santé mentale en hémato-oncologie pédiatrique – Partie 3 : Évaluation de la faisabilité, de l’acceptabilité et de l’appréciation

Abstract


Introduction : Afin de mieux répondre aux besoins des jeunes atteints de cancer et de leur famille, un rôle novateur d’infirmier clinicien en santé mentale (ICSM) en hématologie-oncologie pédiatrique a vu le jour dans un centre hospitalier universitaire pédiatrique du Québec en février 2022. Deux articles précédents expliquent la méthode utilisée pour développer le rôle d’ICSM ainsi que la stratégie d’implantation employée.

Objectifs : La présente étude vise d’abord à évaluer l’efficacité de la stratégie d’implantation utilisée en mesurant sa faisabilité. Dans un deuxième temps, elle vise à explorer l’acceptabilité et l’appréciation du rôle de l’ICSM un an après l’implantation.

Méthode : Trois groupes de participants (adolescents, parents et soignants) ont complété un questionnaire de recherche adapté du Theoretical Framework of Acceptability (TFA), possédant une bonne fidélité interjuges et validité discriminative. Le questionnaire informatisé comprend des affirmations de Likert (1-5) auxquelles ont été ajoutées des questions ouvertes. Une analyse descriptive standard a été utilisée et les réponses obtenues aux questions ouvertes ont permis de soutenir ou de nuancer les résultats quantitatifs.

Résultats : En tout, 31 professionnels de la santé, 10 adolescents atteints de cancer et 10 parents ont rempli le questionnaire. Entre février 2022 et février 2023, un total de 691 interventions ont été réalisées auprès de 112 personnes (patients et parents). De ces 112 personnes à qui une première évaluation a été offerte, 61 se sont fait proposer un suivi clinique; 99,6 % de ces suivis ont été acceptés. Par ailleurs, une très forte corrélation existe entre le nombre d’heures travaillées par l’ICSM et le nombre d’interventions réalisées (r = 0,96). La très forte majorité des participants des trois groupes à l’étude ont mentionné que le rôle est approprié, apprécié et nécessaire, et qu’il apporte une expertise clinique en santé mentale. Les soignants, les adolescents et les parents redemanderaient ce service et le recommanderaient à d’autres si nécessaire. Les résultats indiquent donc une excellente faisabilité, une très bonne acceptabilité et une très bonne appréciation.

Conclusion : La présente étude indique que le rôle implanté contribue à l’amélioration de la prise en charge clinique en santé mentale des adolescents atteints de cancer et des parents. Le rôle amène une expertise nécessaire en soins infirmiers psychiatriques et contribue à l’amélioration de la pratique clinique des soignants. Une prochaine étude serait nécessaire pour mesurer l’efficacité des interventions réalisées par l’ICSM.
