Revue intégrative du soutien psychologique infirmier visant à réduire l’anxiété chez les personnes opérées pour un cancer colorectal
Abstract
Objectifs : Synthétiser les données sur les interventions infirmières qui réduisent l’anxiété chez les personnes opérées pour un cancer colorectal et décrire les principales caractéristiques d’un soutien psychologique efficace.
Contexte : L’anxiété est très fréquente chez les personnes atteintes d’un cancer colorectal durant la période périopératoire, ce qui nuit à la guérison, accroît les complications et diminue la qualité de vie. Plusieurs approches non pharmacologiques et psychologiques ont été étudiées, mais les données sur l’apport des interventions infirmières demeurent insuffisamment structurées.
Devis : Revue intégrative de la littérature.
Méthodologie : Une recension d’articles publiés entre janvier 2020 et juin 2025 a été effectuée dans PubMed et Ichushi-Web, une importante base de données médicales japonaises, pour tenir compte de la recherche propre au Japon et des articles en japonais qui ne sont pas nécessairement indexés dans d’autres pays. Les études retenues examinaient les interventions infirmières visant à réduire l’anxiété ou la détresse psychologique chez les adultes opérés pour un cancer colorectal. La présélection des articles a été effectuée à l’aide des titres et des résumés, puis du texte intégral. Au total, six études répondaient aux critères et ont fait l’objet d’une synthèse narrative sous différentes catégories, dont les interventions non pharmacologiques, les approches fondées sur la psychothérapie, la transmission d’information personnalisée, la collaboration multidisciplinaire et le soutien familial.
Résultats : Les approches non pharmacologiques (relaxation musculaire progressive, musicothérapie, pleine conscience et soins infirmiers périopératoires complets) ont réduit l’anxiété et les symptômes dépressifs. Les approches fondées sur la psychothérapie mises en place environ une semaine avant l’opération se sont montrées particulièrement efficaces. La transmission d’information personnalisée en fonction de l’état psychologique et de la capacité cognitive individuelle a réduit l’incertitude liée à la maladie. Les approches multidisciplinaires et le soutien familial ont favorisé l’adaptation, la prise de décision et la stabilité émotionnelle.
Conclusion : Le soutien apporté par les infirmières réduit l’anxiété périopératoire chez les personnes atteintes d’un cancer colorectal. Les interventions hâtives, la continuité du soutien après le départ de l’hôpital ainsi que les approches multidisciplinaires et celles qui mettent à contribution la famille semblent particulièrement bénéfiques.
