Besoins de formation continue des infirmières pour la prise en charge des patients traités par thérapie CAR-T à la suite d’un cancer hématologique : un sondage canadien

Abstract


La thérapie CAR-T représente une avancée majeure en pleine évolution dans le traitement des cancers hématologiques, mais elle requiert des soins complexes et hautement spécialisés. Les infirmières, qui assurent une part centrale de cette prise en charge, ont donc besoin d’une formation continue adaptée. Or, peu de données probantes existent pour guider le développement de tels programmes. Cette étude présente les résultats d’un premier sondage canadien visant à identifier les besoins de formation continue des infirmières qui soignent des patients adultes recevant un traitement de thérapie CAR-T.

Un sondage bilingue (français et anglais), élaboré à partir du cadre de compétences proposé par He et collaborateurs (2024) et adapté au contexte canadien, a été diffusé auprès des membres de l’Association canadienne des infirmières en oncologie (ACIO/CANO). Les données ont été analysées au moyen des statistiques descriptives et de l’analyse thématique qualitative. 

Au total, 88 répondants ont rempli le questionnaire. La majorité était des femmes (94 %), âgées de 30 à 49 ans (73 %), travaillant principalement en oncologie (38 %) ou en centres ambulatoires (35 %). Les résultats révèlent des besoins importants tant en connaissances théoriques (processus de traitement, complications) qu’en habiletés opérationnelles (administration, gestion des effets secondaires, suivi ambulatoire, communication interdisciplinaire). Des écarts notables apparaissent selon le lieu d’exercice, l’expérience clinique et la formation antérieure. 

Ces constats soulignent la nécessité de développer des programmes de formation continue structurés, accessibles et adaptés aux différents contextes de pratique, afin de soutenir les infirmières dans la prestation de soins sûrs et de qualité aux patients traités par thérapie CAR-T.
