Le pictogramme de la fatigue : évaluer les données psychométriques d’un nouvel outil de dépistage
Abstract
La fatigue est un des effets secondaires du cancer les plus pénibles et pourtant les cliniciens y accordent généralement peu d’attention dans le cadre de leurs rendez-vous cliniques fort chargés. Ce projet visait à évaluer les propriétés psychométriques d’un nouvel instrument conçu pour identifier rapidement les patients qui éprouvent des difficultés liées à la fatigue. Nous avons réalisé l’évaluation auprès d’un groupe hétérogène de 220 patients sous chimiothérapie. Le pictogramme de la fatigue à deux éléments a affiché une bonne fiabilité de test-retest sur une période de 24 heures (coefficient de Spearman de l’ordre de 0,69 pour la question 1 et de 0,72 pour la question 2) et sur le plan de l’équivalence des méthodes (en personne et au téléphone) (coefficient de Spearman de l’ordre de 0,69 pour la question 1 et de 0,59 pour la question 2). Nous en avons évalué la validité en comparant les résultats du nouvel outil à ceux de l’inventaire multidimensionnel de la fatigue et l’échelle FACT-an. Dans l’ensemble, les patients qui affichaient des niveaux de fatigue élevés ont fait état de niveaux élevés sur toutes les échelles respectives.
Le nouveau pictogramme de la fatigue était facile à administrer et à coter dans un milieu clinique fort occupé. Il fournit un instrument standardisé fiable et valide permettant d’identifier les patients qui éprouvent des difficultés liées à la fatigue et d’amorcer une conversation à propos de cet effet secondaire incommodant.
